Massage Therapy Licenses

Massage Enterprise applications require an extensive background investigation and approval by City Council. City Council meetings are the 1st and 3rd Monday of each month. Once City Council has approved the license, the background is complete, and a Certificate of Occupancy has been issued, you will receive your license and may conduct business. 

Massage therapists also require a background check. These backgrounds are usually complete within 2 weeks. When the background is approved, the City Clerk’s office will contact the enterprise or therapist to come in for a photo identification badge.  If the background is not successful the license fee will be refunded but the background fee is non-refundable. Licensed therapists can only work at the enterprise they are licensed to. The City has instituted a cap of 10 for massage enterprises; if a license is not available, applications will be accepted on a first come basis.

Licensing is required to practice therapeutic massage in the City of Blaine.

Requirements for License Application Submittal

The following documents need to be available for upload during the online application process.

Please Note: Documents need to be in a PDF format.

  • Background Investigation Release Form (PDF)
  • Driver's License (color copy front only)   OR
    MN I.D. Card    OR
    Government Issued I.D.
    (color copy of US permanent resident card, passport, etc.)
  • Individual Massage Therapist Professional Liability Certificate of Insurance
    (in the amount of $1 million)
  • Letter of Affiliation
    (signed by massage enterprise owner)
  • National Background Check - Employer may need to set up an account for individual therapist to obtain a National Background Check.  (listed below are national  background investigative providers)

The City of Blaine requires additional information if applying for an Individual Massage Therapist License:

  • Proof of Accreditation
    (mailed directly to the Clerk’s Office from the school)
    • Accredited Institution Official Transcript (CERTIFIED) from an educational institution holding accredited status with the United States Department of Education or Minnesota Office of Higher Education showing a minimum of 500 hours of certified therapeutic massage training with content that includes the subjects of anatomy, physiology, hygiene, ethics, massage theory and research, and massage practice from an accredited program or accredited institution that has been approved by the Issuing Authority. These training hours must be authenticated by a single provider through a certified copy of the transcript of academic record from the school issuing the training, degree or diploma.

    • Accredited Program Proof from educational institution showing a professional massage program is or was accredited by the Commission on Massage Therapy Accreditation (COMTA) at the time of attendance.

Additional information required if applying for a Massage Enterprise License:

  • Minnesota Worker's Compensation Insurance Coverage
  • Legal Description
    Description of the premises to be licensed together with a plan of the area showing dimensions, location of buildings, street access, and parking facilities; include floor number, street number, and all rooms where massage services will be conducted.
